Qualifications

• High school diploma or GED is required.

• Florida Alzheimer’s Disease and Related Dementias Training certification.
• Valid Florida driver’s license and reliable transportation.
• Active BLS for healthcare professionals from AHA or Red Cross.
• Ability to interpret instructions and data, perform calculations, and communicate effectively.

Responsibilities

• Develop and implement homecare services for PACE participants.
• Assess homecare needs of a frail elderly population and create plans of care.
• Collaborate with Interdisciplinary Team to provide quality services.
• Monitor patient, family, and caregiver needs and problems.
• Coordinate and authorize DME, supplies, and home services.
• Enter and maintain patient care, demographics, timesheets, care plans, and reports.
