Cherry Blossom PACE is seeking a Home Care Coordinator to oversee the delivery of home care services for participants. You will collaborate with the Interdisciplinary Team to assess needs, develop care plans, supervise home care staff, and ensure services are delivered safely and in compliance with PACE regulations.

Qualifications

• High school diploma or GED required.
• Current Virginia Certified Nursing Assistant (CNA) license in good standing.
• Current CPR/BLS certification.
• Minimum two (2) years of experience providing direct care to frail or elderly adults in a home care, PACE, long-term care, assisted living, or similar healthcare setting.
• Previous experience serving as a Lead CNA, Team Lead, Home Care Coordinator, Scheduler, or other healthcare leadership role.
• Demonstrated ability to coordinate staff schedules, prioritize participant needs, and communicate effectively with interdisciplinary healthcare teams.
• Valid driver's license, reliable transportation, and acceptable driving record.
