Qualifications

Responsibilities

• Conduct thorough home health assessments and customize care plans.
• Educate patients and families on health maintenance and disease prevention.
• Assist with ADLs with dignity and respect.
• Coordinate schedules with patients per plan of care.
• Monitor symptoms and report changes to physicians.
• Administer medications and treatments as prescribed.
• Document care in EMR accurately.
