Apollo Home Healthcare in Northern California's Bay Area is hiring a Registered Nurse (RN) for full-time or part-time/per diem positions to join our Home Health team. You will provide in-home nursing services, oversee a licensed vocational nurse, and coordinate care with physicians and the clinical team.

Active California RN license and CPR/BLS certification are required. Travel to patients' homes within Alameda, Contra Costa, or Solano counties is expected.

Qualifications

• Active California RN license.
• Current CPR/BLS certification.
• Willingness to travel to patients' homes.
• Reliable transportation.

Responsibilities

• Provide in-home nursing visits according to the physician's plan of care.
• Assess patients' health status, including condition, history, and home environment.
• Develop and update individualized care plans based on patient needs.
• Administer medications and treatments as prescribed.
• Educate patients and caregivers on disease management and home care.
• Monitor patient progress and communicate changes to the physician and care team.
• Document visits and care in a timely manner.
• Coordinate care with therapists, aides, and other team members.
• Serve as case manager for assigned patients and support discharge planning.
