Job summary

Apollo Home Healthcare in Northern California's Bay Area is seeking a registered nurse (RN) to provide in-home nursing care, coordinating with physicians, therapists, and a supportive clinical team. You may work full-time or part-time/per diem to meet patient needs and territory coverage in Alameda, Contra Costa, or Solano counties.

Active California RN license and CPR/BLS are required; home health experience is preferred.

Responsibilities

• Provide in-home nursing visits according to physician's plan of care.
• Assess health status, history and home environment.
• Develop and update individualized care plans.
• Administer medications and treatments as prescribed.
• Educate patients and caregivers on disease management and home care.
• Monitor progress and report changes to physician and care team.
• Document visits and care promptly.
• Coordinate care with therapists, aides and other team members.
• Serve as case manager and support discharge planning.

Apollo Home Healthcare is hiring a full-time or part-time/per diem Registered Nurse (RN) to join our Home Health team, serving patients in Northern California's Bay Area. In this role, you will provide high-quality nursing services to patients in the comfort of their own homes, helping them manage their care and regain independence. You will oversee a supporting Licensed Vocational Nurse and work closely with physicians, therapists, and a supportive clinical team to coordinate care and support patients and their families.

What You'll Do

• Provide in-home nursing visits according to the physician’s plan of care

• Administer medications and treatments as prescribed

• Monitor patient progress and communicate changes to the physician and care team

• Coordinate care with therapists, aides, and other team members
• Serve as case manager for assigned patients and support discharge planning

• Active California RN license

• Experience using Kinnser/Wellsky preferred, but not required

• $40-155/hour
