Job summary

Apollo Home Healthcare, part of Obran Cooperative, is recruiting for a Registered Nurse (RN) to join our Home Health team in Northern California's Bay Area. We offer full-time or part-time/per diem roles with a supportive clinical team and opportunity for membership in Obran Cooperative.

As an RN, you'll provide in-home nursing care, develop patient-specific plans, administer medications, and coordinate with physicians, therapists, and aides to ensure comprehensive patient support.

Qualifications

• Active California RN license is required.
• CPR/BLS certification is required.
• Strong communication and organizational skills are essential.
• Reliable transportation and willingness to travel to patients' homes.
• Home health experience is preferred; Kinnser/WellSky experience is a plus.

Responsibilities

• Provide in-home nursing visits according to the physician’s plan of care.
• Assess patients’ health status, including condition, history, and home environment.
• Develop and update individualized care plans based on patient needs.
• Administer medications and treatments as prescribed.
• Educate patients and caregivers on disease management and home care.
• Monitor patient progress and communicate changes to the physician and care team.
• Document visits and care in a timely manner.
• Coordinate care with therapists, aides, and other team members.
• Serve as case manager for assigned patients and support discharge planning.
