Apollo Home Healthcare is seeking a Registered Nurse (RN) to join our Home Health team in the Northern California Bay Area. We offer full-time or part-time/per diem positions and supervision by a licensed team, with a focus on excellent patient-centered care.

Qualifications

• Active California RN license is required.
• CPR/BLS certification required.
• Strong communication and organizational skills expected.
• Reliable transportation and willingness to travel to patients\' homes.
• Home health experience preferred.
• Kinnser/WellSky experience preferred.

Responsibilities

• Provide in-home nursing visits according to the physician's plan of care.
• Assess patients\' health status, history, and home environment.
• Develop and update individualized care plans.
• Administer medications and treatments as prescribed.
• Educate patients and caregivers on disease management and home care.
• Monitor patient progress and communicate changes to the physician and care team.
• Document visits and care in a timely manner.
• Coordinate care with therapists, aides, and other team members.
• Serve as case manager for assigned patients and support discharge planning.
