Responsibilities

• Provide in-home nursing visits per physician plan of care.
• Assess health status and home environment.
• Develop and update individualized care plans.
• Administer medications and treatments.
• Educate patients and caregivers on disease management.
• Coordinate with therapists and care team.
• Document visits in a timely manner.
• Serve as case manager and assist discharge planning.
