Apollo Home Healthcare, part of Obran Cooperative, is hiring a Registered Nurse to join our Bay Area home health team. You’ll deliver in-home nursing, coordinate care with physicians, therapists, and a supportive clinical team, and support families in regaining independence.

We offer full-time and part-time/per diem roles with flexible scheduling, mileage reimbursement, and opportunities for membership and profit-sharing within the cooperative.

Qualifications

• Active California RN license required.
• CPR/BLS certification required.
• Strong communication and organizational skills.
• Reliable transportation and willingness to travel to patients’ homes.
• Home health experience preferred.
• Kinnser/Wellsky experience preferred if possible.

Responsabilités

• Provide in-home nursing visits according to physician plan of care.
• Assess patients’ health status and home environment.
• Develop and update individualized care plans.
• Administer medications and treatments as prescribed.
• Educate patients and caregivers on disease management and home care.
• Monitor patient progress and communicate changes to the care team.
• Document visits and care in a timely manner.
• Coordinate care with therapists, aides, and other team members.
• Serve as case manager for assigned patients and support discharge planning.
