Qualifikationen

- Home health experience preferred.
Aufgaben
- Provide in-home nursing visits per physician plan of care.
- Assess patients’ health status, history, and home environment.
- Develop and update individualized care plans.
- Administer medications and treatments as prescribed.
- Educate patients and caregivers on disease management and home care.
- Monitor patient progress and communicate changes to the physician and care team.
- Document visits and care in a timely manner.
- Coordinate care with therapists, aides, and other team members.
- Serve as case manager for assigned patients and support discharge planning.
