CarePartners Health Services in Asheville, NC seeks a Home Care Registered Nurse Coordinator for the PACE program to ensure high-quality in-home services. The role focuses on coordinating care teams and resources to support elderly participants in their homes.

You will plan and organize homecare operations, coordinate clinicians and non-skilled aides, assess participants in home environments, and use telehealth to promote wellness and improve outcomes. A valid NC RN license and BSN is preferred.

Qualifications

• BSN required; ADN holders must commit to BSN within 6 years of hire.
• Must be licensed RN in NC with valid driver’s license and reliable transportation.
• Two years on an interdisciplinary team in hospital, nursing home, or home-based setting.

Responsabilités

• Coordinate day-to-day operations of in-home services for PACE participants.
• Act as liaison between home health agency and PACE program; coordinate nurses and aides.
• Assess participants in home environments and plan care with appropriate resources.
• Provide treatments and health education; maintain medical records and documentation.
• Utilize telehealth and community resources to promote health and wellness at home.
