Job summary

Trinity Health PACE seeks a RN‑licensed Nurse Case Manager to coordinate comprehensive health and social services for older adults in the PACE program. You will work with the interdisciplinary team to assess needs, develop plans, and coordinate 24‑hour care in participants’ homes and centers.

The role emphasizes collaboration, regulatory compliance, and education within a mission‑driven, caregiver‑focused environment, with opportunities for leadership and professional growth.

Qualifications

• Graduate of an accredited Nursing program with active RN license.
• CPR certification required.
• At least 2 years of geriatric population experience.
• At least 1 year of frail/elderly care experience or willingness to complete PACE training.
• Valid driver’s license with proof of insurance.

Responsibilities

• Demonstrate and model the Mission, Vision, Values, and Guiding Behaviors of Trinity Health PACE.
• Foster a warm, collaborative environment for participants, families, and colleagues.
• Coordinate assessments, care plans, and 24-hour care delivery with the IDT.
• Conduct home visits and support discharge planning for participants.
• Maintain accurate clinical documentation and ensure regulatory compliance.
• Promote interdisciplinary teamwork and participate in QAPI initiatives.
• Mentor and support team members, contribute to department operations, and manage budget/supplies.
• Provide education and participate in committees, including on-call responsibilities as needed.
