Job summary

CarePartners Health Services in Asheville, NC seeks a Home Care Registered Nurse Coordinator for the PACE program to ensure high-quality in-home care for participants and families. Under the Operations Director, you will manage day-to-day in-home services and coordinate a multidisciplinary team.

You will assess patients in their homes, develop plans of care, provide treatments and education, maintain medical records, and leverage telehealth and community resources to support health at home.

Qualifications

• BSN required; ADN must commit to obtaining BSN within 6 years.
• NC RN license required; valid driver’s license and proof of insurance.
• Two years of interdisciplinary team experience.
• At least 1 year working with frail or elderly population.

Responsibilities

• Coordinate day-to-day In Home Services for participants.
• Liaise between the home health agency and the PACE program.
• Assess participants in their home environment and plan care.
• Provide treatments and health education.
• Maintain medical records and documentation.
• Utilize telehealth and community resources to support health at home.
