Job summary

UnitedHealth Group is seeking a Field Case Manager RN to provide in-home care coordination for Optum Home & Community Care members in Alabama. The role involves assessments, care planning, and collaboration with a multidisciplinary team to address medical, behavioral, and social needs.

You will document in EMR, educate patients and families, and coordinate transitions across health care settings while traveling within 60 miles of your residence. A valid Alabama RN license is required.

Qualifications

• Current unrestricted RN licensure in Alabama.
• 2+ years of relevant nursing experience.
• Experience with complex behavioral, social and/or functional needs.
• Willingness to travel up to 60 miles from residence.

Responsibilities

• Assess health status and establish goals within licensure.
• Plan, implement and evaluate the plan of care.
• Collaborate with multidisciplinary team to address barriers to care.
• Document accurately in electronic medical records (EMR).
• Provide counseling and education to patients and families.
