Job summary

Optum Home & Community Care is hiring an RN Field Case Manager for the Care at Home program in Connecticut. This role delivers in-home visits, telephonic outreach and interdisciplinary team coordination to support a seamless health journey for patients.

Requirements include CT RN licensure, 2+ years of relevant experience, 60-mile travel radius, and a valid driver’s license. Salary range $60,200–$107,400 with a comprehensive benefits package.

Qualifications

• Current unrestricted licensure as RN in Connecticut.
• 2+ years of relevant experience.
• Experience in assessing medical needs of patients with complex behavioral, social and/or functional needs.
• Demonstrated ability to work with diverse care teams in various settings including patient homes.
• Demonstrated solid computer skills, including use of electronic medical records.
• Ability to travel 100% of the time for field-based work within 60 miles of residence.
• Valid driver’s license.
• Access to reliable transportation to travel to client/patient sites within area.

Responsibilities

• Reports to RN Manager.
• Assess health status of members within licensure scope and model frequency.
• Establish goals to meet identified health care needs.
• Plan, implement and evaluate responses to the plan of care.
• Work with multidisciplinary team to address medical, functional, and social barriers to care.
• Collaborate with mental health clinicians to bridge mental and physical health.
• Consult with patient’s PCP, specialists, or other health care professionals as appropriate.
• Assess needs for community resources and make referrals.
• Facilitate transition between care settings in collaboration with primary care physician.
• Document in electronic medical record.
• Provide counseling and education on health maintenance, disease prevention, and follow-up.
