Job summary

Optum Home & Community Care in Alabama is seeking a Field Case Manager RN to join a care team delivering in-home visits and telephonic outreach. The role focuses on assessing health status, developing plans of care, and coordinating services across the care continuum.

You will collaborate with nurse practitioners, behavioral health advocates, and other professionals to bridge gaps between mental and physical health, while documenting in the electronic medical record.

Qualifications

• Current unrestricted RN license in Alabama.
• 2+ years of relevant experience.
• Experience with patients with complex behavioral, social and/or functional needs.
• Ability to work with diverse care teams in home settings.
• Willingness to travel within 60 miles of residence.

Responsibilities

• Assess health status and establish goals to meet identified health care needs.
• Plan, implement and evaluate responses to the plan of care.
• Collaborate with multidisciplinary team to address medical, functional, and social barriers to care.
• Bridge mental and physical health between care settings and teams.
• Document completely in the electronic medical record.
• Provide counseling and education on health maintenance and follow-up.
