Qualifications

• Associate degree or higher in Nursing (RN)
• Active and unrestricted RN license in SC
• Certified in Basic Life Support
• 3+ years of clinical experience
• Ability to move a 30-pound bag and perform physical assessment
• Proficient in Microsoft Office products including Word, Excel and Teams
• Proficient in documenting medical information with written and electronic medical records
• Highly adaptable to changing needs of members daily
• Proven ability to develop and maintain positive customer relationships
• Proven effective communication skills and time management
• Access to reliable transportation and willingness to travel between member locations daily

Responsibilities

• Ensure accurate and complete documentation
• Communicate and collaborate with the interdisciplinary care team
• Support telehealth visits and set up telehealth with the APC
• Assist the provider/team with care coordination activities
• Assist with Medicare benefits determination (Part A, B, D)
• Collaborate with APC and facility to address patient changes
• Assist in therapy coordination
• Participate in facility-based meetings
• Schedule and participate in family conferences and team meetings
• Coordinate training and quality measures
• Assist in documentation and coding, medication reconciliation, nursing rounds
• Write verbal orders in chart per RN scope and laws
• Assist in managing acute or chronic condition events
• Assist APC with transfer documentation for SNF
