Job summary

UnitedHealth Group seeks a Field Case Manager RN to provide in-home care and care coordination across settings, collaborating with a multidisciplinary team. You will assess patient needs, develop plans of care, and educate patients and families in health maintenance and follow-up.

Qualifications

• Current unrestricted RN licensure in Georgia.
• 2+ years of relevant experience.
• Experience with assessing medical needs of complex patients.
• Able to travel 100% within 60 miles of residence.
• Valid driver's license and reliable transportation.

Responsibilities

• Assess health status and establish goals to meet identified health care needs.
• Plan, implement and evaluate responses to the plan of care.
• Collaborate with multidisciplinary team to address medical, functional, and social barriers to care.
• Coordinate referrals to community resources for services.
• Document accurately in the patient’s electronic medical record.
• Support transitions between care settings with the PCP and other clinicians.
