Job summary

ComForCare is seeking a Registered Nurse to perform non-clinical in-home assessments, focusing on conversation, observation, documentation, and care planning without direct bedside tasks. You will travel to clients’ homes to complete assessments and develop practical plans for in-home caregivers to follow.

This role emphasizes building relationships, coordinating care, and using nursing judgment in a community setting, with predictable schedules rather than hands-on clinical duties.

Qualifications

• Active RN license in good standing.
• Experience conducting verbal and observational assessments.
• Strong documentation and care planning skills.
• Ability to work independently in community settings.

Responsibilities

• Travel to clients’ homes for scheduled, non-clinical nursing assessments.
• Conduct verbal and observational assessments with clients and families.
• Review health history, diagnoses, medications, and daily routines.
• Identify risks and non-medical service needs for clients.
• Develop and update individualized care plans for in-home caregivers to follow.
• Maintain accurate documentation of assessments and care plans.
