Job summary

Fox Rehabilitation in Connecticut seeks motivated, licensed Physical Therapist Assistants to deliver one-on-one care in patients’ homes and senior living communities. You’ll work under a supervising PT to implement individualized plans and guide functional activities for older adults.

You will join an interdisciplinary team, enjoy mentorship, and build your clinical skills through FOX’s Geriatric House Calls model, with flexible scheduling and meaningful patient relationships in a supportive,

Qualifications

• Valid Physical Therapist Assistant license in the state(s) of practice, or eligibility to obtain
• Graduate of an accredited Physical Therapist Assistant program
• Current CPR certification
• Basic computer skills; proficient in Microsoft Office

Responsibilities

• Deliver individualized treatment plans established by your supervising Physical Therapist
• Instruct, motivate, and safeguard patients through therapeutic exercises and functional activities
• Document visits accurately and on time within FOX’s EMR system
• Coordinate coverage with patients, POAs, and referral sources
