Job summary

FOX Rehabilitation is seeking a Physical Therapist Assistant to deliver in‑home and senior living care under the supervision of a Physical Therapist. You will implement individualized treatment plans, document care in FOX’s EMR, and collaborate with an interdisciplinary team to support patient independence.

Qualifications

• Valid Physical Therapist Assistant license in the state(s) of practice, or eligibility to obtain.
• Graduate of an accredited Physical Therapist Assistant program.
• Current CPR certification.

Responsibilities

• Deliver individualized treatment plans established by supervising PT, adjusting activities based on progress.
• Instruct, motivate, and safeguard patients through therapeutic exercises and functional activities.
• Document visits accurately in FOX’s EMR system per clinical standards.
• Stay connected with patients, POAs, and referral sources; help coordinate coverage when needed.
