Job summary

Fox Rehabilitation is seeking licensed Physical Therapist Assistants in Connecticut for in-home and community-based patient care. You will work under a supervising PT to implement customized treatment plans, guide patients through exercises, and document progress in FOX’s EMR system.

This role emphasizes one-on-one care in patients’ homes and senior living communities, with mentorship from supervising PT and opportunities to grow clinical skills and caseload.

Qualifications

• Licensed Physical Therapist Assistant in CT or eligibility to obtain license.
• Graduate of an accredited PTA program.
• Current CPR certification.
• Basic computer skills; proficient in Microsoft Office.

Responsibilities

• Deliver individualized treatment plans established by supervising PT.
• Instruct, motivate, and safeguard patients through exercises and activities.
• Document visits accurately in FOX EMR per standards.
• Coordinate coverage and stay connected with patients, POAs, and referral sources.
