Job summary

Fox Rehabilitation in West Hartford, CT is seeking motivated, licensed Physical Therapist Assistants to provide hands-on, in-home care to older adults. You’ll work with a supervising Physical Therapist to deliver individualized treatment plans, guide patients through exercises, and document progress in FOX’s EMR.

Qualifications

• Must be licensed or eligible for PTA licensure in CT
• CPR certification required
• Proficient in basic computer skills and MS Office

Responsibilities

• Deliver individualized treatment plans established by supervising PT
• Instruct and guide patients through therapeutic exercises and activities
• Document visits accurately in FOX EMR following Clinical Documentation standards
• Coordinate with patients, POAs, and referral sources for coverage when needed
