Job summary

FOX Rehabilitation in Stamford, CT seeks a licensed Physical Therapist Assistant to deliver one-on-one, in-home and senior living care under a supervising PT. You will implement individualized treatment plans and guide patients through exercises within FOX’s Geriatric House Calls framework.

Qualifications

• Valid Physical Therapist Assistant license in the state of practice, or eligibility to obtain
• Graduate of an accredited Physical Therapist Assistant program
• Current CPR certification
• Basic computer skills; proficient in Microsoft Office

Responsibilities

• Deliver individualized treatment plans established by supervising Physical Therapist
• Instruct, motivate, and safeguard patients through therapeutic exercises
• Document visits accurately and on time in the EMR
• Coordinate with care team and community referral sources as needed
