Job summary

FOX Rehabilitation in Connecticut seeks licensed Physical Therapist Assistants to deliver in-home, one-on-one rehabilitation under the supervision of a Physical Therapist. You will implement individualized treatment plans and guide patients through therapeutic activities in their homes and senior living environments.

Qualifications

• Valid Physical Therapist Assistant license in CT or eligibility.
• Graduate of an accredited PTA program.
• Current CPR certification and basic computer skills.

Responsibilities

• Deliver individualized treatment plans established by your supervising Physical Therapist.
• Instruct, motivate, and safeguard patients through therapeutic exercises and functional activities, tracking progress.
• Document visits accurately in FOX’s EMR system per Clinical Documentation standards.
• Stay connected with patients, POAs, and referral sources, coordinating coverage when needed.
