Job summary

FOX Rehabilitation in Milford, CT is seeking motivated Physical Therapist Assistants to provide hands-on, in-home care as part of the Geriatric House Calls model. You will work one-on-one with older adults, delivering individualized plans under supervision, guiding exercises and activities in patients' homes and communities.

You'll join a supportive, interdisciplinary team with mentorship from supervising PT, growing your skills, caseload, and career through structured pathways and flexible

Qualifications

• Valid PTA license in the state(s) of practice, or eligibility to obtain.
• Graduate of an accredited PTA program.
• Current CPR certification.
• Basic computer skills; proficient in Microsoft Office.

Responsibilities

• Deliver individualized treatment plans established by supervising PT, adjusting activities based on patient response and progress.
• Instruct, motivate, and safeguard patients through therapeutic exercises and functional activities, observing and tracking progress.
• Document visits accurately and on time within FOX's EMR system, following Clinical Documentation standards.
• Stay connected with patients, POAs, and community referral sources, and help coordinate coverage when needed.
