Job summary

Lifespan Therapy seeks a Home Health Physical Therapist to provide in-home geriatric care across California, delivering comprehensive evaluations and tailored plans of care. You will focus on mobility, strengthening, gait, and fall prevention, working independently with flexible scheduling.

Qualifications

• Valid California Physical Therapist license.
• CPR or BLS certification.
• Driver's license and auto insurance.
• Physical + TB test (within the last year).
• Professional liability and business license (if applicable).

Responsibilities

• Conduct comprehensive in-home evaluations and develop individualized plans of care for geriatric patients.
• Focus on functional strengthening, gait training, balance, and fall risk reduction to improve patient mobility and independence.
• Schedule and conduct home visits based on your availability and patient needs.
• Work with a supportive care team, including other disciplines and supervisors, to ensure coordinated patient care.
• Use one simple EMR system for all notes (with tools available to minimize documentation time!).
