Qualifications

• Valid California Physical Therapist license required.
• CPR or BLS certification.

Responsibilities

• Implement interventions to improve mobility, balance, and gait.
• Schedule and conduct home visits based on availability.
• Collaborate with care team to ensure coordinated patient care.
• Document notes using a single EMR system to minimize time spent on documentation.
