Qualifications

• Valid California Physical Therapist license required.
• CPR or BLS certification.
• Driver's license and auto insurance.
• Physical + TB test (within the last year).
• Professional liability and business license (if applicable).

Responsibilities

• Provide Skilled PT care with in-home evaluations and individualized plans of care for geriatric patients.
• Implement therapeutic interventions focusing on strengthening, gait training, balance, and fall prevention to improve mobility.
• Manage your caseload by scheduling and conducting home visits based on availability and patient needs.
• Collaborate with a supportive care team to ensure coordinated patient care.
