Job summary

Lifespan Therapy is hiring a Home Health Physical Therapist (PT) to provide in-home geriatric care across California. You will evaluate patients, create individualized plans, and deliver functional interventions with autonomy and flexibility.

Qualifications

• Valid California Physical Therapist license.
• CPR or BLS certification.
• Driver's license and auto insurance.
• Physical + TB test (within the last year).
• Professional liability and business license (if applicable).

Responsibilities

• Provide Skilled PT Care: Conduct comprehensive in-home evaluations and develop individualized plans of care for geriatric patients.
• Implement Therapeutic Interventions: Focus on functional strengthening, gait training, balance, and fall risk reduction.
• Manage Your Caseload: Schedule and conduct home visits based on your availability and patient needs.
• Collaborate for Optimal Outcomes: Work with a supportive care team to ensure coordinated patient care.
• Document Efficiently: Use one simple EMR system for all notes.
