Job summary

Lifespan Therapy is seeking a Home Health Physical Therapist to deliver in-home geriatric care across California. You will perform comprehensive evaluations, create individualized plans, and guide patients toward improved mobility in a flexible, autonomous schedule.

The role emphasizes safe, evidence-based interventions, collaboration with a supportive team, and efficient documentation using TherapySync EMR. California license and transport are required.

Qualifications

• Valid California Physical Therapist license.
• CPR or BLS certification.
• Driver's license and auto insurance.
• Physical + TB test (within the last year).
• Professional liability and business license (if applicable).

Responsibilities

• Provide Skilled PT care through in-home evaluations and develop individualized plans of care for geriatric patients.
• Implement functional strengthening, gait training, balance, and fall risk reduction.
• Schedule and perform home visits based on availability and patient needs.
• Collaborate with a care team to ensure coordinated patient care.
• Document efficiently using a single EMR system and keep notes timely.
