Job summary

Lifespan Therapy is seeking a Home Health Physical Therapist to provide expert in-home care for geriatric patients across California. You will assess, treat, and develop personalized plans of care in patients' homes, prioritizing safety, function, and independence.

You will manage your caseload with flexible hours, collaborate with a multidisciplinary team, and use TherapySync EMR for streamlined documentation and billing. California licensure and CPR/BLS are required.

Qualifications

• In-home PT care for geriatric patients in California.
• Develop and implement individualized plans of care.
• Coordinate with care teams to ensure continuity of care.

Responsibilities

• Provide Skilled PT Care: conduct in-home evaluations and develop individualized plans of care for geriatric patients.
• Implement Therapeutic Interventions: focus on strengthening, gait training, balance, and fall risk reduction.
• Manage Your Caseload: schedule and conduct home visits based on availability and patient needs.
• Collaborate for Optimal Outcomes: work with a supportive care team to ensure coordinated patient care.
• Document Efficiently: use EMR system to record notes and minimize documentation time.
