Job summary

Lifespan Therapy is hiring a Home Health Physical Therapist to provide in-home evaluations and develop patient-centered plans of care for geriatric clients across Southern California. You will implement interventions to improve mobility, balance, and independence while managing your own caseload with a flexible schedule.

Qualifications

• Valid California Physical Therapist license.
• CPR or BLS certification.
• Driver’s license and auto insurance.
• Physical + TB test (within the last year).
• Professional liability and business license (if applicable).

Responsibilities

• Provide Skilled PT care: conduct in home evaluations and develop individualized plans of care for geriatric patients.
• Implement therapeutic interventions: functional strengthening, gait training, balance, fall risk reduction.
• Manage caseload: schedule and conduct home visits based on availability and patient needs.
• Collaborate with a care team and supervisors to ensure coordinated patient care.
