Lifespan Therapy is seeking licensed Home Health Physical Therapists to deliver patient-centered care in California, covering Santa Clara and San Jose areas. You’ll operate within our app-based network, managing your own caseload with high autonomy and flexibility.

Qualifications

• Valid California Physical Therapist license.
• CPR or BLS certification.
• Driver's license and auto insurance.
• TB test within the last year.
• Professional liability and business license (if applicable).

Responsibilities

• Provide Skilled PT Care: conduct in-home evaluations and develop individualized geriatric plans.
• Implement Therapeutic Interventions: focus on functional strengthening, gait, balance, and fall risk reduction.
• Manage Your Caseload: schedule and conduct home visits based on availability.
• Collaborate for Optimal Outcomes: work with care team to ensure coordinated patient care.
