Millennium Physician Group is seeking a House Calls Advanced Practice Provider (AP) to deliver complex clinical management to patients in home settings. The APP collaborates with the Medical Director to ensure timely, patient-centered care and adherence to evidence-based practices.

The role requires NP licensure, board certification in an applicable family or adult-gerontology track, and experience in primary or ambulatory care.

Qualifications

• Master's or Doctor of Nursing Practice from an accredited program.
• Current, unrestricted NP license in the applicable state.
• Board certification as FNP/AGPCNP/AGACNP or related.
• DEA registration and BLS required; ACLS preferred.

Responsabilités

• Provide in-home clinical management to meet patient needs and reduce ED visits.
• Participate in after-hours on-call telephonic support (6–10 shifts/year).
• Document conditions in EHR promptly and in compliance with CMS and insurers.
• Identify patients needing additional services and refer to home health or community resources.
