Responsibilities

• Evaluate, diagnose, and manage patients with acute illnesses, chronic diseases, and preventive care needs through history, physical examination, and evidence-based decision-making.
• Develop individualized treatment plans including medication management, preventive services, immunizations, and chronic disease management.
• Coordinate referrals to specialty care and allied health professionals as needed; support transitions of care and care coordination.
• Document encounters and outcomes in the EHR, maintain regulatory compliance, and participate in quality improvement initiatives.
