Responsibilities

• Evaluate, diagnose, and manage patients with acute illnesses, chronic diseases, and preventive care.
• Develop individualized treatment plans including meds, preventive services, and immunizations.
• Order and interpret labs, imaging, ECGs, and point-of-care testing.
• Perform office-based procedures such as wound care, drainage, injections.
• Conduct annual wellness visits and chronic care management.
• Coordinate referrals and collaborate with the healthcare team.
• Educate patients and caregivers on diagnoses, medications, and self-management.
