Job summary

Millennium Physician Group seeks a Nurse Practitioner, General / Primary Care, to provide comprehensive ambulatory primary care across the lifespan. The APP will independently practice within the NP scope, evaluating and treating acute and chronic conditions, with collaboration to physicians as needed.

Responsibilities include orders, care planning, education, and preventive services, all supported by evidence-based guidelines and a focus on high-quality, patient-centered care.

Qualifications

• Requires Master's-level NP training within the APP scope.
• Escalates beyond-scope matters to collaborating physicians.
• Ambulatory primary care experience is typical for APPs.

Responsibilities

• Evaluate, diagnose, and manage acute illnesses, chronic diseases, and preventive care.
• Develop treatment plans including medication management, preventive services, immunizations, and screenings.
• Order and interpret labs/imaging to support clinical decisions.
• Perform office-based procedures (minor wound care, injections, etc.).
• Conduct annual wellness visits and preventive screenings.
• Coordinate referrals and collaborate with a multidisciplinary care team.
• Educate patients and caregivers on diagnoses and treatment plans.
