Millennium Physician Group seeks a House Calls Nurse Practitioner to deliver in-home, patient-centered care in Collier County, FL. The APP will manage complex conditions, coordinate with leadership and medical directors, and provide evidence-based treatment in a home setting.

Qualifications

• Master's or doctoral NP program completed.
• Current, unrestricted NP license in the applicable state.
• NBN certification such as FNP/AGPCNP/AGACNP required or appropriate.
• DEA registration and BLS certification current.
• ACLS preferred.
• 2+ years of advanced practice in primary/family/internal care.

Responsabilités

• Provide in-home clinical management and patient-centered care.
• Participate in after-hours telephonic on-call rotations.
• Document all conditions timely in EMR per CMS and MPG policies.
• Refer to home health, social services, or community resources when needed.
• Collaborate with interdisciplinary team to optimize patient outcomes.
