Job summary

Millennium Physician Group seeks a House Calls Advanced Practice Provider to deliver complex clinical management for homebound patients. The APP collaborates with the Medical Director to provide patient-centered, evidence-based care and participates in the on-call rotation with telephonic support.

Requirements include an NP degree, an unrestricted license, and board certification in FNP/AGPCNP/AGACNP or related field. Strong communication and care coordination are essential.

Qualifications

• Master's or Doctor of Nursing Practice from an accredited NP program.
• Unrestricted NP license and nursing licensure in the applicable state.
• Board certification as FNP, AGPCNP, AGACNP or equivalent.

Responsibilities

• Provide clinical medical management in home settings to meet patient needs.
• Participate in on-call rotation with after-hours telephonic support.
• Document all active medical conditions in the EMR per policy and CMS rules.
• Identify patients needing additional services and arrange referrals as needed.
• Contribute to quality measures and HCCs to meet goals.
