Millennium Physician Group is seeking a Primary Care Advanced Practice Provider (APRN) to deliver patient-centered care across the continuum. You will work with physicians and a multidisciplinary team to diagnose, treat, and manage acute and chronic conditions, with emphasis on quality outcomes and value-based care.

This role involves documentation, care coordination, and leadership in patient care planning.

Competenze

• Master's or Doctor of Nursing Practice degree from an accredited Nurse Practitioner program required.
• Current, unrestricted Nurse Practitioner license and Advanced Practice Registered Nurse licensure in the applicable state required.
• National board certification as FNP/AGPCNP/AGACNP or other applicable certification required.
• DEA registration and BLS certification required.
• ACLS certification preferred.
• Two or more years of advanced practice clinical experience in primary care strongly preferred.

Mansioni

• Total care of all assigned patients in collaboration with the care team.
• Perform patient assessments, care planning, risk management, and provide acute and chronic care services within scope.
• Prevent unnecessary Emergency Department utilization when appropriate.
• Document all conditions in the EMR timely and in compliance with CMS and payer requirements.
• Provide leadership in nursing processes to improve outcomes and coordinate interdisciplinary projects.
• Identify patients needing additional assistance and refer to home health or community resources.

Formazione
