Job summary

Millennium Physician Group is seeking a Primary Care Advanced Practice Provider (APRN) to deliver patient-centered care across the continuum. You will work with physicians and an interdisciplinary team to assess, diagnose, treat, and coordinate services for adults and aging patients in a value-based care setting.

You will drive quality outcomes through preventive care, documentation, and collaboration with care teams, home health, and community resources to enhance patient outcomes.

Qualifications

• Master’s or Doctor of Nursing Practice from an accredited program.
• Current NP license and APRN licensure in the applicable state.
• Board certification in FNP/AGPCNP/AGACNP or equivalent.
• DEA registration and BLS certification are required.
• ACLS preferred; two+ years of primary care experience recommended.
• Knowledge of primary care guidelines, chronic disease management, and value-based care.
• Experience with EHRs and health IT supporting quality care and compliance.

Responsibilities

• Provide total care for assigned patients with a collaborative care team.
• Conduct assessments, plan care, and manage acute and chronic conditions.
• Prescribe medications and interpret diagnostics as appropriate.
• Coordinate with home health, specialists, and community resources.
• Support preventive care, care gap closure, and documentation compliance.
• Lead in care planning and guideline-based, patient-centered care.
