Job summary

Abode Care Partners, an affiliate of BrightSpring Health Services, seeks a Nurse Practitioner to provide primary care in seniors’ homes and related communities. You will perform clinical assessments, develop individualized care plans, and collaborate with interdisciplinary teams to improve outcomes.

Qualifications

• Active state license for NP/PA or PA license with RN in state employed.
• MSN required for Nurse Practitioners; PA may not need a master's.
• Current CPR and DEA licensure in good standing.
• Experience with EMR systems and office productivity tools.
• Strong communication and professional collaboration skills.

Responsibilities

• Performs medical care planning, directing and evaluating patient needs.
• Treats chronic and episodic illnesses, injuries, and urgent conditions.
• Orders and interprets labs and diagnostics as needed.
• Collaborates with caregivers and health professionals for a comprehensive plan of care.
• Ensures up-to-date medical records and referrals when necessary.
