Abode Care Partners, affiliated with BrightSpring Health Services, seeks a Nurse Practitioner to deliver primary care in seniors’ homes and complex care patients. You will assess patients, develop care plans, and coordinate with interdisciplinary teams in skilled nursing, assisted living, and independent living communities.

Qualifications

• Active state Nurse Practitioner license.
• Current NP National Board Certification.
• Active state DEA licensure.
• MSN degree (Master of Science in Nursing).
• Five+ years nursing experience; at least two years NP experience preferred.
• Strong interpersonal communication skills - verbal and written.
• Excellent time management with accountability for daily workflow.
• Proficiency with medical instruments and equipment.
• Strong knowledge of HIPAA and OSHA regulations.
• Experience with electronic medical records (EMR) and Microsoft Office.

Responsabilités

• Performs evidence-based medical care by planning, directing, and evaluating patient medical needs.
• Examines and treats chronic and urgent episodic illnesses, minor injuries, accidents, and other injuries or illnesses.
• Orders and interprets laboratory and diagnostic testing as needed.
• Collaborates with caregivers and other healthcare professionals in providing interdisciplinary care and maintaining a comprehensive plan of care.
• Provides referrals to other healthcare practitioners when necessary.
• Ensures patient medical records are kept up to date.
