You will perform assessments, develop personalized care plans, and collaborate with interdisciplinary teams to ensure seamless transitions and preventative care.

Qualifications

• Active Nurse Practitioner license in the state
• Current NP National Board Certification
• Active state DEA licensure
• Master's degree in Nursing (MSN)
• Five+ years nursing experience; NP experience preferred
• Strong interpersonal and communication skills
• Excellent time management and accountability
• Proficiency with medical instruments and equipment
• Knowledge of HIPAA/OSHA regulations
• Experience with EMR and Microsoft Office
• Licensed driver with automobile insurance
• Ability to engage in moderate physical activity

Responsabilités

• Performs evidence-based medical care by planning, directing, and evaluating patient medical needs
• Examines and treats chronic and urgent illnesses, minor injuries, and other conditions
• Orders and interprets laboratory and diagnostic tests as needed
• Collaborates with caregivers and other healthcare professionals to maintain a comprehensive care plan
• Provides referrals to other healthcare practitioners when necessary
• Ensures patient medical records are up to date
