Job summary
Abode Care Partners, an affiliate of BrightSpring Health Services, seeks a Nurse Practitioner to deliver primary care in seniors’ homes. You will assess patients, develop care plans, and work with interdisciplinary teams in skilled nursing, assisted living, and independent living communities.
The role emphasizes autonomy, preventative care, and building lasting patient relationships within home-based settings.
Qualifications
- Active state NP license.
- Current NP National Board Certification.
- Active state DEA licensure.
- Master's degree in Nursing (MSN).
- Five or more years of nursing experience; at least two years of NP experience preferred.
Responsibilities
- Performs evidence-based medical care by planning, directing, and evaluating patient medical needs.
- Examines and treats chronic and acute illnesses and minor injuries.
- Orders and interprets laboratory and diagnostic testing as needed.
- Collaborates with caregivers and other healthcare professionals in providing interdisciplinary care and maintaining a comprehensive plan of care.
- Provides referrals to other healthcare practitioners when necessary.
- Ensures patient medical records are kept up to date.
