Job summary

Abode Care Partners, an affiliate of BrightSpring Health Services, seeks a compassionate Nurse Practitioner to provide primary care to seniors and patients with complex conditions in home settings.

You will perform clinical assessments, develop personalized care plans, and collaborate with interdisciplinary teams to improve outcomes. We offer enhanced compensation with quarterly performance incentives and a comprehensive benefits package.

Qualifications

• Active state NP license.
• Current NP board certification.
• Active state DEA licensure.
• Master’s degree in Nursing (MSN).
• Five or more years of nursing experience; at least two years NP experience preferred.
• Strong interpersonal communication skills – verbal and written.
• Excellent time management skills with accountability for practitioner’s daily workflow.
• Proficiency in medical instruments and equipment.
• HIPAA and OSHA regulations knowledge.
• Proficiency in computer-based data management programs, EMR systems, and Microsoft Office.
• Licensed driver with valid automobile insurance.
• Ability to engage in moderate physical activity (up to 25 lbs) and stand/walk long periods.

Responsibilities

• Provides primary care to seniors and patients with complex health needs in home settings.
• Performs clinical assessments and develops personalized care plans.
• Collaborates with interdisciplinary teams to ensure seamless care transitions.
• Orders and interprets laboratory and diagnostic testing.
• Provides referrals to other healthcare practitioners when necessary.
• Maintains up-to-date patient medical records.
