Abode Care Partners is seeking a Nurse Practitioner to deliver comprehensive primary care in seniors’ homes and community settings. You will assess, plan, and manage healthcare for patients in skilled nursing, assisted living, and independent living environments.

Qualificações

• Active state license as Physician Assistant or Nurse Practitioner.
• MSN required for NPs; PA may not require a master.
• Current CPR certification.
• Active state DEA licensure.
• 5+ years nursing healthcare experience; 2+ years NP/PA experience preferred.
• Strong interpersonal and written communication skills.
• Good time management and daily workflow accountability.
• Proficiency with EMR and Microsoft Office.
• Valid driver's license and car insurance.

Responsabilidades

• Provide evidence-based primary care in homes and facilities.
• Assess and manage chronic and acute conditions.
• Order and interpret labs and diagnostics.
• Collaborate with caregivers and interdisciplinary teams.
• Provide referrals when needed.
• Maintain accurate medical records.
