Job summary

Abode Care Partners is seeking a comprehensive primary care Nurse Practitioner to provide in-home services to seniors and patients with complex medical needs. The NP will conduct clinical assessments and develop personalized care plans in skilled nursing, assisted living, and independent living communities.

Qualifications

• Active state NP license required.
• Current NP national board certification.
• Active state DEA licensure.
• Master's degree in Nursing (MSN).
• Five+ years nursing experience; NP experience preferred.
• Strong interpersonal communication, verbal and written.
• Excellent time management, daily workflow accountability.
• Proficiency with medical instruments and equipment.
• Knowledge of HIPAA and OSHA regulations.
• Experience with EMR and MS Office.
• Hold a valid driver's license and auto insurance.
• Ability to engage in moderate physical activity.

Responsibilities

• Plans, directs, and evaluates patient medical needs.
• Treats chronic and episodic illnesses and minor injuries.
• Orders and interprets lab and diagnostic tests.
• Collaborates with interdisciplinary care teams.
• Provides referrals to other practitioners when needed.
• Maintains up-to-date patient records.
