Qualifications

• Active NP license in the state.
• Current NP board certification.
• Active DEA licensure.
• Master’s degree in Nursing (MSN).
• Five+ years nursing experience; at least two years NP experience preferred.
• Strong interpersonal and written communication skills.

Responsibilities

• Provide primary care to seniors and patients with complex needs in home/community settings.
• Conduct clinical assessments and develop personalized care plans.
• Coordinate with interdisciplinary teams for seamless care transitions.
• Manage chronic conditions and acute visits with preventive focus.
• Order and interpret labs and diagnostic tests as needed.
• Maintain accurate patient records and documentation.
